What is Metformin?

Metformin is a biguanide drug that has been used for type 2 diabetes for over 60 years. It is now one of the most studied anti-aging compounds, with the landmark TAME (Targeting Aging with Metformin) trial underway. Epidemiological data shows diabetics on Metformin live longer than non-diabetics not on Metformin — a remarkable finding that sparked the longevity research.

Mechanism of Action

Metformin primarily inhibits Complex I of the mitochondrial electron transport chain, reducing ATP production and activating AMPK (the cellular energy sensor). AMPK activation mimics caloric restriction, activates autophagy, reduces mTOR activity, improves insulin sensitivity, and reduces inflammatory signaling (NF-κB).

Metformin Dosing Protocol

Anti-aging: 500–1,000 mg/day oral (much lower than diabetes doses of 1,500–2,550 mg/day). Take with food to minimize GI side effects. Some longevity researchers use 500 mg before bed only. Note: Metformin may blunt exercise adaptations — consider timing around workouts.

Does Metformin blunt exercise benefits?
This is controversial. Some studies show Metformin reduces exercise-induced mitochondrial adaptations and muscle gains. Others show no effect.
